Community Action's Supportive Housing Program is a two-year transitional housing program that provides intensive, in-home and group-based case management to participants. 

The program specifically serves individuals who are: 1.) Pregnant, have custody of their children, or have a reunification plan within six months; 2.) Verifiably homeless; 3.) At least 18 years of age. Those served through the Supportive Housing Program must all be willing to participate in intensive case management designed to lead them on a path toward long-term self-sufficiency and keep them in stable housing.

Am I Eligible?

To Be Eligible:

1. Applicant must be pregnant, have children, and have custody of those children, or have a reunification plan within six months.
2. Applicant must have identifiable case management needs and be willing to follow a goal plan.
3. Applicant must be verifiably homeless.
4. Applicant must be at least 18 years old.
